It has been loosing tentacles don't have time to post any paramours atm, I know ammonia 0, and nitrite 0, tank is 4 months old. Pics:

I have never seen a frogspawn lose a tentacle per say.

If your frogspawn is disintegrating, losing heads, you are not able to do much about it now.

When those things start dying they just don't stop.

Only think you can do is crack the dead heads off and hope for the best.

Idk about losing multiple tentacles but ive seen the occasional tip of a hammer floating in my tank figured a fish picked it off or something if theres a bunch dropping somethings gotta be wrong

Is it a mixed reef? I've had a leather coral get mad and nuke my tank. Parameters remained stable, but the leather poisoned all the corals downstream from it.
If you can't diagnose the problem you can try a big water change and a bunch of carbon.
