Wiring Metal Halide ballast

Hey all, I bought a pair of used 250w icecap metal halide ballasts recently and they didnt have any power cords or sockets attached to them. three questions,

To wire the ballast to a plug, I know green is ground so that goes to the ground prong. but which one of the flat prongs does the white go to?

Going to the bulbs, i have blue and yellow, on my sockets are white black and green. Green gets grounded to the shell of the ballast but does black go to blue or yellow?

I got a pair of 250w phoenix bulbs, theyll work with these ballasts right? A friend told me they would but i wanted to double check.
