Help! Shroom melt!

This was a beautiful sized ricordea that out of nowhere started melting with no change in location or waterflow after 3 months.
Salinity, ph, amonia, nitrates, trites, ca, alk, mg all in check...any ideas. Phosphates are at .05.
Anything i can do.

Try to move it to a better flow area with less light, it may have been getting sick over the last few months. Or something changed you might have had a spike in temperature if only for a day and that did it in. You never know it might come right back.
