Can some ID this one?

Its hard to tell but the body is light green and the tips are light blue. The Polyps are green/yellow in color.

Hard to tell it looks like if it was bleaching too...what kind of lights are you using?

the frag is under LEDs (~216 watts) the tank is a 75 gallon. I slowly moved it up from the sand bed over about 2 months. When I got the frag it was light brown and as I moved it up it change colors.

Here is my water parameters:

SG 1.025
Mg 1380 (Salifert test kit)
Ca 420 (API test kit)
Kh 9 (API test kit)
Nitrate 5 (API test kit)
Phosphates not 0 but less then .25 (API test kit need to buy a better test kit)
Ammonia 0 (API test kit)
Nitrite 0 (API test kit)
