I just found a lot of these on the glass of my nano. Nothing has been introduced to the tank for at least 4 months. I added hermits and snails then. Last night I did scrape a mushroom off the back of the tank with a razor and put it on a frag disc.

That' looks like flatworm red planearia . Salifert Exit will get rid of it.

Flatworms, not sure which ones but they are flatworms.

Strange thing is that the only think I have added to the nano in the past 12 months have been hermits and snails. I've never seen them in there before.

Ugh, they're not red so they're not red planeria flatworms. Might be the white flatworms which feed on Acros.
