I agree, looks like ich....
is he alone in the tank? how long have you had him?
number 1 thing, make sure hes eating A LOT. If he stays eating a lot he will be ok but then you have to work on getting the ich out of the tank...
my opinion, get a strong UV light and start running it.
again, number 1 thing, make sure that fish keeps eating.
Im sure youll get 100 other replies on things to try.

Hard to tell from pic but Does not look like ich to me, looks like lymphosystis but on the body.

Maybe leave alone and monitor for few days? If it is lymph it will clear up on its own with better water conditions.
