When do I remove the old pellets? Or do I just keep on topping up as they grind down to smaller bits?

I have the opposite experience, po4 way down, nitrates still high. I would suggest at least 40% water changes a week to bring it down.

I know that it is essential to test before dosing. So go for salifert test kits, alternatively have your water tested by your LFS once a week. You can also ask a friend to test for you.
